Sign your TELUS Kroll activation request

Fill in your pharmacy details, review the terms, and sign below. When you submit, your signed request goes straight to the Kramer team — and you'll get a PDF copy for your records.

(the “Pharmacy Client”) is hereby requesting TELUS Health and Payment Solutions Limited Partnership (“TELUS”) to activate the Feature in the TELUS' Kroll pharmacy management system (“Kroll”) to the following pharmacies, which Feature will improve the process of transferring compound prescriptions from the originating pharmacies to the Kramer network of compounding pharmacies.
